General Sleep Apnea Equipment Insurance Claim Instructions
Before you begin your sleep apnea equipment claim process, please note that CPAP Store USA is only able to provide you with a general overview of the insurance claim process.
Depending on your insurance company, the process can vary, so be sure to consult with your insurance provider that these are the steps you need to follow to make a successful claim.
Contact Your Insurance Company
As you start the claim process, you will want to start by contacting your insurance company’s customer service and asking if you are allowed to submit a claim on your own behalf.
Not all insurance providers allow people to bill them for out-of-pocket medical purchases, so it would be good to know that prior to filling out any paperwork.
Inquire About Needed Claim Forms
As you talk to your insurance customer service representative, be sure to ask if there are any particular claim forms you will need to submit. If the company does have a particular form, have them email or fax it to you while you are still on the phone with them. That way, you can make sure you receive the forms you need.

Fill Out Insurance Form With Billing Codes and Info
Fill out the insurance form required by your providers. You will need billing code information as well as other insurance codes so that your insurance providers can process your claim. We will have the Healthcare Common Procedure Coding System (HCPCS) information you need in the section below called “Insurance Billing Codes For Sleep Apnea Equipment”.
Attach CPAP Store USA Invoice
To prove that you have purchased qualifying sleep apnea equipment, attach a copy of your CPAP Store USA invoice. You will also need the invoice to know what HCPCS billing codes you will need to fill out, so be sure to keep the original invoice handy.
Submit Claim Information
Submit your required claim information. Your insurance company should have a specific method of submission, whether mail, fax, or email.
From there, you will have to wait as your claim is processed. Most companies take between 30-60 days. Your insurance provider should be able to give you a relatively accurate claim turnaround time.
